Our professional staff understands that every person’s health care needs are different. To ensure that the special needs of each resident are properly met, we take an individualized approach to our nursing care and rehabilitative services.

Dedicated to meeting the needs of the whole person our staff designs and individual care plan based on a resident’s admission assessment. A team of professionals representing our medical, nursing, social service, and dietary departments meet with the resident, his her family members and physician to determine the level of care required. On the basis of these meetings and assessment we develop a personal care plan.

The plan is reevaluated every three months through consultation with the resident, family members and they physician. Continuous assessment and communication help us ensure that holistic treatment is maintained and that the level of care meets the resident’s current needs.
